Objective

• you are able to plan, carry out and assess interventions to promote health and functional ability on an individual, group and community level using a multi-specialist and cross-disciplinary network
• you are able to plan, carry out and assess nursing in national health care on an individual, group and community level at health centers and home health care using a multi-specialist and cross-disciplinary network

Content

• Assessment of health, functional ability and health habits
• Recognition of the need for a change, determination of targets and agreement on methods to achieve the targets
• Principles of monitoring and assessment of outcome and impact on health
• Exercise
• Psychosocial factors
• Effective methods of promoting health and functional ability, including rehabilitation
• Treatment of the most common chronic diseases at health centers and in home (health) care within the nurse's area of operation (cardiovascular diseases, musculoskeletal diseases, osteoporosis, mental illness and substance abuse, cancer, infectious diseases, asthma and allergies, respiratory diseases, diabetes, dementia, oral and dental diseases, ear nose and throat diseases)
• Work in multispecialist and cross-disciplinary networks

Qualifications

Professional competence of a nurse responsible for general nursing (180 ECTS credits).
Patient clinical assessment (5 ECTS credits)
