Preclinical training in basic nursing interventions (5cr)

Objective

You can monitor and assess patients' conditions systematically.

Content

How to use the ABCDE protocol to assess patients' wellbeing?

Study forms and methods

Training in Hoitotaitojen perusosaamisen harjoittelu: in-school training. The topics and content material for the practice days can be found in the Learn module of the Hoitotaitojen perusosaaminen -course. In-school practice is part of the EU nurse's statutory compulsory clinical training. Every hour of training is compulsory.


An accelerated learning pathway:
Students who have completed a previous professional qualification in health care and are working can complete the practical part of the course through the recognition and validation of competences. A single day of demonstration is allocated for this purpose at the beginning of the course during the orientation week. Instructions and a link for registration will be provided during the orientation week and/or in an electronic message sent by Xamk to new students.

Kinesthetics and Ergonomics (0,5 credits):
You can apply for admission if you have a valid Patient Transfers Ergonomics Card 3 credits. The kinesthetics component is completed with a written assignment.

Documentation
Students who have completed a previous professional qualification in health care and are working can complete the practical part of the documentation through the recognition and validation of competences (exam).

RDI and work-related cooperation

The basic nursing skills exercises use examples from working life in patient case studies and simulation exercises.

Possible expert lectures or working life visits.

Clinical Practicum 1 (work-based training) is 6 credits. This is a separate implementation.

Timing of exams and assignments

Assessment of Hoitotaitojen perusosaaminen exercises: assessment of skills in injections, blood sampling and cannulation during practice sessions. In addition, assessment of overall competence of about 1.5h per student at the end of the course.

International cooperation

Not included.

Student workload

Total workload 5 x 27h = 135 h. Campus days (1 - 2) about every two weeks. The school offers the opportunity to practice nursing activities independently.

Nursing skills (guided + independent):
Compulsory guided practice at school 7 x 7 - 8 hrs/student.
Optional refresher training under the supervision of a teacher approx. 8h.
Competency assessment approx. 1,5h/student.
Independent practice at the school.

Contents of the exercises

Kinesthetics and ergonomics skills:
* Functional capacity assessment and activation
* Rehabilitation support
* Patient-safe transfers
* Kinesthetics in nursing

Aseptic procedures (theory part in the second course)
- Basics of asepsis
- Hand hygiene, personal hygiene, cleaning, waste management
- Precautions in patient care
- Identifying the risk of infection
- Sterile gloves


Basic nursing care and prevention of pressure ulcers
- Basic care assistance methods and aids
- Assisting with personal hygiene, pressure ulcer prevention.

Basics of wound care

Assessment and support of respiration
- Monitoring and examination of breathing, assessment of airway openness
- Respiratory failure, dyspnoea, risk of aspiration
- Observation of clinical signs
- Oxygenation equipment and mask ventilation
- Aspiration of mucus from airways and tracheostoma

Urinary tract catheterisation and bowel function promotion
- Monitoring urination, maintaining urinary tract function (catheterisation and urine opening)
- Monitoring and promoting bowel function (listening to bowel sounds, emptying bowels, treating bowel obstruction)

Care of the dying patient
- Recognising signs of impending death
- Caring for the dying patient, putting the deceased to bed and meeting relatives

Fluid therapy
- Preparation of antibiotic infusion (theory in Lääkehoito ja näyttöön perustuva hoitotyö -course)
- Cannulation

Nutritional assistance
- Enteral nutrition and tube feeding
- Monitoring and assessing nutritional status, ensuring nutritional intake

Assessment of level of consciousness (Avpu, GCS)

Examination and assessment of circulation and cardiac function, basic resuscitation
- Palpation of heart rate and temperature limits
- Measurement of blood pressure
- Monitoring
- ECG
- Basic resuscitation and defibrillation
- First aid

Medical treatment
- S.c. and i.m. injections (theory in Lääkehoito ja näyttöön perustuva hoitotyö)

Taking blood samples
- Capillary sampling
- Venous blood sampling

Other assessment of the patient's condition
- Systematic assessment of the patient's condition according to the ABCDE protocol, assessment of the patient's condition according to the News criteria and reporting according to the Isbar model

Basic caring skills: participation in compulsory caring skills exercises at school. At the end of the course, assessment of competence: pass/fail.

Absences from compulsory exercises will be a barrier to participation in the assessment of competence and to the completion of the course.
