Objective

Purpose of the course In this course, you will learn the basics of planning and implementing a goal-oriented case management process that supports the client's participation and resources. Competences Client relationship competence in social services, health care and rehabilitation Promoting well-being and agency Service system competence Objectives of the course Once you have completed this course, you will be familiar with the concept of case management and its theoretical starting points and significance. You can describe and analyse different models and processes of service guidance implementation; you can build a client-oriented service path and analyse the critical and development points of service paths.

You can define case management as a concept, explain the theoretical models of service guidance and its significance You can describe the main features of the case management process and the legislation defining service guidance You are familiar with and know how to describe building a client-oriented service path and describe the development points of case management
